Practice registration

The form asks for practice, location, service, ownership or control, pricing-readiness, and work contact details. These are used for review, verification, and follow-up before a listing is published. Registration does not automatically authorize publication of specific prices.

Messages and launch notifications

The contact form collects your name, email, and message so the team can respond. The notification form collects an email and signup time for patient-launch updates. By selecting “Notify me,” you agree to receive these updates. You can unsubscribe anytime. Do not include medical records, patient identifiers, or other health details in either form.

Storage and service providers

Practice registrations are saved in private Vercel storage and copied to a private Google backup and Google Sheets review workflow. Contact messages and notification signups are saved in private Vercel storage. Launch emails are not sent automatically at signup.

Drafts on your device

The registration form can save a draft in your browser so you can return to it. The contact form temporarily keeps an unfinished message in browser session storage. Use a device you trust, and clear saved drafts when you no longer need them.

Website analytics

We use Vercel Web Analytics to count visitors and page views. We also save anonymous interaction counts in private Vercel storage, including navigation clicks, registration clicks, form starts, progress, and confirmed submissions. Interaction tracking uses a random identifier kept in browser session storage, expiring after 30 minutes of inactivity. It does not collect typed answers, email addresses, phone numbers, medical details, or private management links. Vercel visitor counts are estimates and do not identify you by name.

Your choices

You can cancel a notification signup using the cancellation control shown after signup or the private management link. Cancellation stops future notifications; it does not erase the original consent record. For questions, corrections, or a deletion request, contact the team.
